Our group has identified poor medical adherence as a key problem among patients suffering from chronic illnesses. It is difficult to perform medication reconciliation as the patient may not be aware of the medicine they are taking, or they may not remember if they have taken the medicine. In addition, purchasing medical resupplies can cause inconvenience to the patient.

With these in mind, our solution aims to:

1. Remind patients to take their medication on time
2. Assure caregivers and pharmacists if medications are taken on time, which also aids medication reconciliation
3. Keep track of medication supplies and provide timely alerts when supplies are low
4. Proceed with medication resupply order requests for caregivers upon approval
